Referral
Initial Referral
Contact us directly via phone or email with relevant background information on your client. We aim to respond to all case management referrals within 24 hours and can typically commence assessment within two weeks.

Assessment
Comprehensive Assessment
A thorough neurological and functional assessment is conducted in the patient’s home. We evaluate mobility, balance, upper limb function, cognition, fatigue, and participation — producing a detailed baseline report.

Planning
Treatment Planning
A personalised rehabilitation programme is developed in collaboration with the patient and case manager. Goals are SMART, time-bound, and aligned with the client’s personal priorities and functional needs.

Treatment
Ongoing Treatment
Regular home visits deliver evidence-based neurological physiotherapy. Progress is monitored continuously and treatment adapted as needed. Case managers receive regular written updates at agreed intervals.

Review
Review & Discharge
A formal review at agreed milestones includes updated outcome measures and revised goals. Discharge reports summarise progress, ongoing needs, and recommendations for continued management.
